Overview

This review analyzed studies on the effects of hormone replacement therapy (HRT) on cognitive function in postmenopausal women. While HRT showed small and inconsistent effects on verbal memory, reasoning, and information processing in healthy aging women, larger effects were observed in epidemiological studies. This difference might be related to healthier women choosing HRT. HRT's effects may depend on age, type of menopause, and specific HRT used, with Premarin showing the least impact. The effects are independent of mood and menopausal symptom improvement. HRT seems to protect against Alzheimer's disease, but further investigation is needed. Three studies showed no benefit of Premarin in preventing further cognitive decline in women with Alzheimer's disease. Longer-term HRT may even have negative effects in Alzheimer's. Future research should investigate different HRT preparations, estrogen levels, and interactions with age, menopause, and risk factors for cognitive decline and Alzheimer's disease.
