Tailoring Antiobesity Medications Based on Phenotypes: A New Approach to Weight Management

Obesity is a complex and multifaceted health challenge, and finding effective treatments requires understanding individual differences. This study investigated the potential of phenotype-guided treatment for obesity, utilizing biological and behavioral markers to identify distinct obesity phenotypes and tailor medication selection accordingly. The researchers enrolled 450 individuals with obesity and measured various factors including body composition, resting energy expenditure, satiety, satiation, eating behavior, affect, and physical activity. These measurements were used to classify participants into four obesity phenotypes: hungry brain, emotional hunger, hungry gut, and slow burn. In a 12-month pragmatic trial, 312 patients were randomly assigned to receive either phenotype-guided or non-phenotype-guided treatment with antiobesity medications.

The results demonstrated that the phenotype-guided approach significantly enhanced weight loss compared to the non-phenotype-guided group. Patients in the phenotype-guided group experienced an average weight loss of 15.9% compared to 9.0% in the non-phenotype-guided group, highlighting the potential for personalized treatment strategies to improve weight management outcomes.
