Chest Pain in Cardiac Amyloidosis: A Challenging Terrain

Cardiac amyloidosis (CA), a rare but serious condition characterized by the accumulation of amyloid protein in the heart, can significantly impact cardiovascular function. This study delves into the prevalence, causes, and prognostic significance of chest pain in patients with CA. The researchers analyzed data from 174 patients with CA, including 104 with transthyretin amyloidosis (ATTR) and 70 with light-chain amyloidosis (AL). The study revealed that patients with chest pain were more likely to have a history of coronary artery disease (CAD) and heart failure (HF) symptoms. They also had higher levels of high-sensitivity troponin I (hs-cTnI) and brain natriuretic peptide (BNP), markers of cardiac damage and stress. Among patients with chest pain, obstructive CAD was detected in 38% of those undergoing coronary artery imaging, with ATTR-CA being the most common subtype. The study found that vascular/perivascular amyloid deposition was detected in a majority of AL-CA patients undergoing endomyocardial biopsy. During a follow-up period, chest pain was a significant predictor of HF hospitalization, even after adjusting for CA subtype and CAD.
