Author: ChanP, HuangT Y, KoJ T, LeeY S, LinT S, TomlinsonB
Original Abstract of the Article :
This study compared the incidence of cough with the angiotensin-converting enzyme (ACE) inhibitor lisinopril and the diuretic metolazone with angiotensin II receptor antagonist losartan in elderly hypertensive patients with previous histories of ACE inhibitor-induced cough. A randomized, double-blin...See full text at original site

Losartan vs. Lisinopril vs. Metolazone for Elderly Hypertensive Patients
Navigating the vast landscape of hypertension treatment options can be a challenge, especially when dealing with the unique needs of elderly patients. This double-blind comparison study focused on the incidence of cough, a common side effect of angiotensin-converting enzyme (ACE) inhibitors. The researchers embarked on a rigorous journey to compare the effects of lisinopril (an ACE inhibitor), losartan (an angiotensin II receptor antagonist), and metolazone (a diuretic) on elderly patients with a history of ACE inhibitor-induced cough.
Losartan Emerges as a Promising Alternative
The study, conducted across multiple clinics and countries, included 84 elderly patients who had previously experienced cough with lisinopril. The results revealed that losartan was associated with a significantly lower incidence of cough compared to lisinopril. In fact, the cough frequency was almost 80% lower for losartan! Metolazone showed similar results to losartan, indicating a potential alternative for patients who cannot tolerate ACE inhibitors.
Implications for Managing Hypertension in Elderly Patients
This study highlights the importance of considering individual patient factors when choosing antihypertensive medication. Losartan's efficacy in reducing cough incidence offers a valuable treatment option for elderly patients who have experienced this adverse effect with ACE inhibitors. It's crucial to consult with a healthcare professional to determine the best course of treatment for each individual, taking into account their medical history, preferences, and potential risks.
Dr.Camel's Conclusion
Losartan, like a cool desert oasis providing respite from the harsh sun, can be a welcome alternative for elderly hypertensive patients who experience cough with ACE inhibitors. This research offers valuable insights into individualizing treatment strategies, helping us to better manage hypertension in this vulnerable population.
