Overview

This review examined the evidence surrounding the optimal time to initiate dialysis treatment. It found that late referrals to dialysis programs were associated with worse outcomes, including higher rates of comorbidity and noncompliance. The review also highlighted the association between renal function at initiation of dialysis and both nutritional status and subsequent clinical outcomes, including survival. While a randomized clinical trial is needed to conclusively determine the optimal initiation time, evidence suggests that initiation of dialysis at a Ccr of 9 to 14 mL/min is justified if there is evidence of malnutrition.
