Overview

A 54-year-old woman with a history of mitral valve prolapse and migraines was admitted to the hospital for substernal chest pain. Electrocardiogram showed ST-segment elevation and positive troponin I, suggestive of an acute coronary syndrome. However, coronary angiogram revealed normal coronary arteries. The patient had been taking zolmitriptan 2-3 times daily for several days before the event due to a persistent migraine headache. She was diagnosed with Takotsubo syndrome (TS), also known as broken heart syndrome. Extensive literature search revealed multiple cases of coronary artery vasospasm secondary to zolmitriptan, but none were associated with TS.
