Overview

A study of 33 patients with severe, treatment-resistant acne compared isotretinoin to placebo. The placebo group saw a 57% increase in cystic lesions, while those who switched to isotretinoin after the placebo period experienced a 98% improvement. Patients who received isotretinoin from the start had a 95% improvement. A total of 27 out of 32 patients treated with isotretinoin achieved complete clearance. The average maximum dose of isotretinoin was 1.2mg/kg/day. Eighteen patients received one 4-month course, while 15 received two courses. All patients are now in remission for an average of 38 months. Biopsies and sebum measurements during treatment showed a significant reduction in sebaceous gland size and function, suggesting that this may be the key mechanism of action for isotretinoin in treating acne.
