Overview

A review of 53 studies on radiotherapy for prostate cancer found no clear evidence to support treatment for early-stage, well-differentiated tumors. Radiotherapy or surgery may be indicated for poorly differentiated tumors, but there is no difference in effectiveness between these two treatments for early-stage tumors. Radiotherapy is generally less invasive than surgery. The choice between surgery and external radiotherapy for more advanced tumors (T1 and T2) is complex, with some patients better suited for one treatment or the other. Radiotherapy is a proven treatment option for more advanced tumors (T3) and is valuable for palliative care in the most advanced stages (T4). The type of treatment should consider the tumor's differentiation grade, with radiotherapy becoming more beneficial as the tumor becomes less differentiated. Post-operative radiotherapy has not been proven effective, and interstitial brachytherapy alone appears less effective than other methods. Combining interstitial and external radiotherapy requires further investigation.
