Major findings
Doripenem is a new carbapenem antibiotic. This study evaluated the safety and tolerability of intravenous doripenem in 1 phase 2 and 6 phase 3 clinical trials conducted with patients with nosocomial pneumonia, including ventilator-associated pneumonia; complicated intra-abdominal infection; and complicated urinary tract infection. Safety data were available from 1817 patients who received doripenem and 1325 patients who received 1 of 4 active comparator drugs as part of this development program. Overall, intravenous doripenem was found to be safe and well tolerated, demonstrating a safety profile comparable to that of comparator agents and a limited propensity to induce seizures, including when administered via 1-h or 4-h infusion. 1 "> 1
Reason for side effects
Carbapenems are generally considered safe drugs, but the main concern is the potential for treatment-emergent seizures. However, doripenem showed negligible neurotoxic effects in preclinical testing. 1 "> 1
Common side effects
Seizures
Doripenem has been shown to have a lower potential to cause seizures compared to other carbapenem antibiotics. 1 "> 1
Gastrointestinal side effects
Doripenem administration has been reported to cause gastrointestinal side effects such as nausea, vomiting, and diarrhea. 1 "> 1
Allergic reactions
Doripenem, like other carbapenem antibiotics, has the potential to cause allergic reactions. 1 "> 1
Others
Doripenem may also cause side effects such as headache, dizziness, and rash. 1 "> 1
Side effects management
Seizures
Doripenem dosage may need to be adjusted in patients at risk for seizures. 1 "> 1
Gastrointestinal side effects
If gastrointestinal side effects are severe, doripenem administration should be discontinued. 1 "> 1
Allergic reactions
If an allergic reaction is suspected, doripenem administration should be discontinued and appropriate treatment should be given. 1 "> 1
Others
If other side effects are severe, doripenem administration should be discontinued. 1 "> 1
Comparison between studies
Commonalities
These studies were conducted on patients with various types of infections to evaluate the safety of doripenem. 1 "> 1
Differences
Each study differed in the type of infection targeted and the type of comparator drugs used. 1 "> 1
Things to keep in mind for real-life application
Doripenem is used to treat serious infections such as nosocomial pneumonia, intra-abdominal infection, and urinary tract infection. 1 "> 1
Before taking doripenem, talk to your doctor about any allergies you may have and any other medications you are taking. 1 "> 1
Take doripenem as directed by your doctor. 1 "> 1
Limitations of current research
These studies did not evaluate the long-term safety of doripenem. 1 "> 1
Future research directions
Further research is needed to evaluate the long-term safety and effectiveness of doripenem. 1 "> 1
Conclusion
The results of these studies suggest that doripenem is a safe and well-tolerated carbapenem antibiotic. 1 "> 1
However, like other antibiotics, doripenem carries the risk of side effects. 1 "> 1
It is important to discuss the risks and benefits of doripenem with your doctor before considering its use. 1 "> 1
