While major endocrine ablation therapy (oophorectomy, adrenalectomy, hypophysectomy) was once common for advanced breast cancer, the use of tamoxifen, a less toxic antiestrogen, has become more prevalent. This study found that major endocrine ablation therapy remained effective as a second-line treatment for advanced breast cancer, particularly in ER-positive cancers, even after tamoxifen resistance developed. A randomized study of adjuvant endocrine and chemotherapy for operable breast cancer, stratified by ER status, showed that tamoxifen alone (20mg/day for 2 years) had a similar recurrence rate to other treatment groups in ER-positive patients.
