[Pyrazinamide versus ethambutol in short-term therapy of lung tuberculosis. A randomized study].

Overview

In a study of 113 people with advanced tuberculosis, treatment with a combination of rifampicin, isoniazid, and pyrazinamide (PZA) for 2-3 months, followed by rifampicin and isoniazid for 9 months, was compared to treatment with rifampicin, isoniazid, and ethambutol (EMB) for the same duration. Patients treated with PZA showed a faster conversion to negative cultures (though not statistically significant) and no relapses after 18 months. Two patients treated with EMB did not respond to therapy. Drug-induced hepatitis occurred in 5 patients on PZA and 2 on EMB, but was reversible. Hepatitis was more common in patients over 70 years old. Three patients on PZA experienced joint pain due to elevated uric acid levels. These preliminary results suggest that PZA may be a safe and effective treatment option for tuberculosis, especially when used with caution in older patients.
