Overview

Combination antiretroviral therapy is the optimal approach for managing HIV infection. Currently, five nucleoside analog reverse transcriptase inhibitors, two nonnucleoside reverse transcriptase inhibitors, and four protease inhibitors are approved for combination therapy in the United States. Antiretroviral therapy is recommended for patients with a viral load higher than 5,000 to 20,000 copies per mL, regardless of CD4+ count. Careful consideration should be given to the patient's history of antiretroviral use, potential side effects, and drug interactions. Nonnucleoside reverse transcriptase inhibitors and protease inhibitors should only be used in combination therapy due to the risk of viral resistance.
