Major Research Findings

Acute myeloid leukemia (AML) is a heterogeneous disease, with a variety of mutations and mechanisms of leukemogenesis. 33 , 38 This heterogeneity makes it difficult to develop effective treatments for all patients. Standard treatment approaches, such as high-dose multi-agent chemotherapy regimens, can provide predictable outcomes for many patients, but they can also fail in specific cases. Moreover, the unselective effect of chemotherapy can lead to high treatment-related toxicity and an increased risk of infection during prolonged pancytopenia, preventing further dose escalation.

Many studies have investigated new treatments to improve the prognosis of AML patients. These include allogeneic stem cell transplantation, 28 , 22 targeted therapies, 55 , 51 and combinations of different drugs.

Long-term outcome of postremission chemotherapy for adults with acute myeloid leukemia using different dose-intensities.

This study evaluated different post-remission chemotherapy regimens for acute myeloid leukemia (AML) patients. Patients receiving intensive ablative maintenance (IM) had a median disease-free survival of 12.4 months, while those receiving intensive, short-term consolidation (IC) had a median of 18.4 months, both significantly better than the no-treatment group (3.3 months). Median survival was 20 months for IM, 47 months for IC, and 5.4 months for no further treatment. While there was a trend towards improved long-term survival with BMT in the IM group, long-term survival was similar for IC regardless of BMT. The study concludes that increasing the intensity of post-remission therapy progressively improves remission duration and survival in AML patients.

A prospective evaluation of the roles of allogeneic marrow transplantation and low-dose monthly maintenance chemotherapy in the treatment of adult acute myelogenous leukemia (AML): a Southwest Oncology Group study.

In a study of 522 people with acute myeloid leukemia (AML), 57% achieved complete remission (CR). Those younger than 50 years old had a higher remission rate (70%). 39% of patients had a suitable donor for a bone marrow transplant, and two-thirds of those received a transplant in their first remission. 5-year disease-free survival was similar for those who received a transplant, those with a donor who did not receive a transplant, and those without a donor (41%, 42%, and 29%, respectively). Monthly maintenance therapy was associated with prolonged disease-free survival, but not improved overall survival. Factors associated with improved overall survival included younger age, lower white blood cell count at diagnosis, having a specific type of leukemia (M3 morphology), and being white. People with M3 AML had particularly favorable outcomes, with 75% disease-free survival and 56% overall survival at 7 years.

For those under 46 with a suitable donor, allogeneic bone marrow transplant (alloBMT) resulted in a higher 3-year disease-free survival rate (66.5%) compared to those who did not have a suitable donor and received either autologous stem cell transplant (ASCT) or chemotherapy (42.4%). AlloBMT was also associated with a lower relapse rate. For patients without a suitable donor, there was no significant difference in outcomes between ASCT and chemotherapy. These findings suggest that alloBMT may be the most effective treatment option for eligible patients with AML.

This study compared two chemotherapy regimens (ara-C with either daunorubicin or aclarubicin) for treating acute myeloid leukemia (AML) in 174 patients. After a follow-up of up to 10 years, there was no significant difference in survival rates between the two regimens. Five-year survival for the group who achieved complete remission was 35%, with younger patients (17-39 years) having a significantly higher survival rate (57%) compared to older patients (40-60 years). While 7 secondary cancers were diagnosed, only 4 were potentially related to the chemotherapy. No increased risk of solid tumors directly caused by the AML treatment was observed. However, there was a general increased risk of secondary cancers in patients who had cancer at the time of their AML diagnosis.

In a study of 517 people with acute myeloid leukemia (AML) under 40 who achieved remission, three intensive treatment options were compared: allogeneic bone marrow transplant (alloBMT), autologous bone marrow transplant (ASCT), and intensive chemotherapy (ICC). While alloBMT was associated with better outcomes for those with a suitable donor, there was no significant difference in survival between ASCT and ICC. The study found that a second round of chemotherapy after a first round of ICC was less toxic than ASCT, with similar survival rates. However, there was no difference in survival between the groups after 5 years. Adding granulocyte colony-stimulating factor (G-CSF) to support the immune system did not improve remission rates or survival. Similarly, there was no difference in survival, relapse, or disease-free survival between those who received three cycles of chemotherapy versus six cycles. Giving interferon for 12 months did not improve outcomes. These findings suggest that the choice of initial chemotherapy regimen for AML is important, but extending chemotherapy beyond three cycles or using interferon does not appear to benefit patients.

While treatment for children with acute megakaryoblastic leukemia (AMKL) has improved, there are still uncertainties about the best treatment and who is at higher risk. This study examined 97 children with AMKL and found that their survival rate was lower (60%) compared to other types of leukemia (68%). While survival rates improved in more recent treatment groups, stem cell transplants didn't significantly improve survival. Certain genetic changes were associated with better or worse outcomes, but the best predictor of success was how well the child responded to initial treatment. AMKL remains a challenging form of leukemia for children, but new treatments have significantly increased survival rates.
