Major Research Findings

Almotriptan is an effective treatment for acute migraine in adolescents and children, according to several studies. 20 , 12 , 6 These studies have shown that almotriptan is superior to placebo in relieving headache pain, achieving pain-free status, and improving associated symptoms, including reduced migraine recurrence. 24 , 26 , 16 Almotriptan has also been shown to be effective for menstrual migraine, which often affects women who experience migraines. 26 , 17 Furthermore, compared to other triptans, almotriptan appears to achieve pain relief and pain-free status more quickly. 19 , 25

This review provides an updated assessment of the evidence for pharmacological therapies used to treat acute migraine attacks. The study focused on medications available from 1998 to 2013 and evaluated their effectiveness based on the level of evidence. Triptans and dihydroergotamine are effective (Level A). Ergotamine and other forms of dihydroergotamine are likely effective (Level B). Acetaminophen, nonsteroidal anti-inflammatory drugs (NSAIDs), opioids, sumatriptan/naproxen, and acetaminophen/aspirin/caffeine are effective nonspecific medications (Level A). Ketoprofen, intravenous and intramuscular ketorolac, flurbiprofen, intravenous magnesium, and the combination of isometheptene compounds, codeine/acetaminophen, and tramadol/acetaminophen are likely effective (Level B). Antiemetics prochlorperazine, droperidol, chlorpromazine, and metoclopramide are also likely effective (Level B). While opioids like butorphanol, codeine/acetaminophen, and tramadol/acetaminophen are probably effective, they are not recommended for regular use due to their potential side effects.
