5-aminosalicylate (5-ASA) agents are the mainstay treatment for ulcerative colitis (UC). Several oral 5-ASA agents are available, including azo-bond pro-drugs like sulfasalazine, olsalazine, and balsalazide, as well as delayed- and controlled-release forms of mesalazine. Achieving good compliance with oral therapy is crucial for its effectiveness, but frequent daily dosing and a high pill burden can negatively impact compliance. Poor adherence poses a significant barrier to successfully managing UC. Newly developed once-daily mesalazine formulations, including the multi-matrix delivery system and mesalazine granules, have been proven effective in inducing and maintaining remission in mild-to-moderate UC. These formulations have a good safety profile comparable to other oral mesalazine formulations and offer the advantage of a lower pill burden. This may contribute to increased long-term compliance, treatment success in clinical practice, and potentially reduce the risk of UC-associated colon cancers. This systematic review summarizes the available literature on the short- and medium-term efficacy and safety of these new once-daily mesalazine formulations.
