Main research findings

Pivmecillinam has been shown to be effective in treating various infections such as acute cystitis, enteric fever, pneumonia, and pyelonephritis in multiple studies. 25 , 11 , 18 , 2 It has been used alone or in combination with other antibiotics, often demonstrating effectiveness. 20 , 17 , 4 , 9 , 7 , 5 , 3 , 8 , 27 , 16 , 10 , 12 , 6 , 19 , 22 , 24 , 21 , 26 , 13 , 23 Pivmecillinam is generally well-tolerated with few side effects reported. 25 , 11 , 15 , 18 , 20 , 14 , 17 , 4 , 2 , 9 , 7 , 5 , 3 , 8 , 27 , 16 , 10 , 12 , 6 , 19 , 22 , 24 , 21 , 26 , 13 , 23 However, some patients have reported experiencing side effects.

Reasons for side effects

Side effects of pivmecillinam can be caused by various factors, including the drug's mechanism of action and individual patient variations. Pivmecillinam works by inhibiting bacterial cell wall synthesis, but this mechanism can potentially affect human cells as well, leading to side effects.

Common side effects

Gastrointestinal

The most common side effects of pivmecillinam are gastrointestinal symptoms. 15 , 17 , 9 , 5 , 16 , 23 Nausea, vomiting, and abdominal pain have been reported. 15 , 17 Some patients have also reported experiencing diarrhea or constipation.

Skin

Skin rash and urticaria, which are allergic reactions, have been reported as side effects of pivmecillinam. 2 , 21 These symptoms are believed to be caused by allergic reactions to pivmecillinam.

Other

Other side effects reported with pivmecillinam include headache, dizziness, and fever. 4 , 14 , 6 Additionally, fungal infections such as candidiasis have been reported. 17 These symptoms are believed to be caused by changes in the intestinal flora due to pivmecillinam, making it easier for fungi to proliferate. 17

Side effect management

Gastrointestinal

If you are concerned about gastrointestinal side effects, try to eat small, frequent meals. 15 , 17 Avoid high-fat foods and stimulants, and eat easily digestible foods. 15 Consult a doctor if symptoms are severe.

Skin

If you develop a rash or urticaria, stop taking pivmecillinam and consult a doctor. 2 , 21

Other

If you are concerned about headache or fever, consult a doctor. 4 , 14 If you suspect a fungal infection like candidiasis, consult a doctor and receive appropriate treatment. 17

Pivmecillinam has been shown to be effective in treating various infections in numerous studies. Furthermore, pivmecillinam has been shown to be generally well-tolerated with few reported side effects.

A fixed dose combination of pivmecillinam and pivampicillin was as effective as co-trimoxazole in treating 12 patients with acute typhoid fever. All clinical isolates were sensitive to both treatments. No side effects were recorded.
