Major Research Findings

Clozapine is an effective treatment for a portion of people with schizophrenia (SZ) who are resistant to the beneficial effects of other antipsychotic drugs. 13 , 4 , 9 , 10 , 7 , 3 , 5 , 6 , 11 , 2 , 1 , 12 , 8 However, a significant proportion of people taking clozapine experience persistent symptoms, even with adequate doses. This group is known as ‘clozapine-resistant’ and comprises 40-60% of patients. 13 Researchers are exploring potential agents that could be used alongside clozapine to enhance its effectiveness without increasing the burden of adverse effects. 13 N-Acetylcysteine (NAC) is one such candidate agent, showing promise in earlier pilot studies. 13 Clozapine is superior to haloperidol in treating both positive and negative symptoms in stable outpatients with schizophrenia. 4 Furthermore, clozapine demonstrates superior antidepressant effects compared to quetiapine and comparable effects to olanzapine and risperidone in chronic schizophrenia, regardless of the presence of a major depressive episode. 9 Despite its effectiveness, clozapine has been associated with potentially life-threatening side effects, including agranulocytosis and myocarditis. 8
