Major Research Findings

Aromatase inhibitors, such as anastrozole and letrozole, are very effective at reducing estrogen production in postmenopausal women and are the most effective endocrine treatments for hormone receptor-positive breast cancer in these women. 12 3 These medications are classified as non-steroidal drugs like anastrozole and letrozole, and steroidal drugs like exemestane. 15 19 2 Multiple clinical trials have shown that anastrozole is more effective at extending disease-free survival in breast cancer than tamoxifen. 8 4 13 9 20 6 Anastrozole is also associated with a higher incidence of side effects such as joint pain and bone pain compared to tamoxifen, but it has a lower incidence of vaginal bleeding and discharge. 13 20 Combining anastrozole with tamoxifen may reduce the effectiveness of tamoxifen. 16 Anastrozole may lead to bone density loss, but studies have shown that it does not increase the risk of osteoporosis. 14 21 There is no clear conclusion on whether anastrozole is better than letrozole. 17 Anastrozole inhibits estrogen more strongly than tamoxifen but has not been shown to negatively impact the quality of life for patients. 17 Anastrozole has been shown to be a more effective treatment for early-stage breast cancer than tamoxifen. 13

Bone density loss

Extended anastrozole therapy (6 years) was not associated with a higher incidence of osteoporosis compared to shorter therapy (3 years). Anastrozole use was associated with a modest decrease in bone mineral density, partially reversible after anastrozole cessation.
