Major Research Findings

Multiple studies have shown that antiresorptive agents, such as hormone replacement therapy and bisphosphonates, are effective for preventing and treating postmenopausal osteoporosis. 24 Bisphosphonates are potent inhibitors of bone resorption and are currently the mainstay of treatment for postmenopausal osteoporosis. 33 Bisphosphonates are widely reported to be generally safe and effective; however, concerns have been raised about potential excessive suppression of bone turnover during long-term use, which can impair some of the biomechanical properties of bone. 48 In the treatment of osteoporosis, it is important to choose the right treatment for each individual's situation. 65

Treatment Summary

Treatment for osteoporosis includes hormone replacement therapy, bisphosphonates, teriparatide, denosumab, raloxifene, and other medications. 24 In addition, calcium and vitamin D intake and exercise therapy are also effective. 103

This study of 45 postmenopausal women with osteoporosis and vertebral fractures compared three treatment groups: calcitonin and calcium, calcitonin, calcium, and phosphate, and calcitonin, calcium, and norandrostenolone decanoate. Calcitonin alone effectively prevented further bone loss for 24 months. Phosphate supplementation was not beneficial. The combination of calcitonin and norandrostenolone decanoate extended bone-preserving effects for 36 months. This suggests that the combination of calcitonin and norandrostenolone decanoate may be a promising and cost-effective treatment for established postmenopausal osteoporosis.

A study of 30 postmenopausal women with osteoporosis compared the effects of alendronate (a bisphosphonate medication) to placebo over six months. Alendronate significantly reduced bone turnover markers within three months and increased lumbar bone mineral density by 3.7% after six months. These positive effects were maintained for six months after stopping treatment. The control group showed a slow decrease in lumbar bone density, which was significant only after 18 months. Femoral bone density was not significantly affected by alendronate, while the control group showed a significant decrease in femoral neck density. This suggests that alendronate may be an effective alternative to estrogen treatment for postmenopausal osteoporosis, particularly in improving lumbar bone density and preventing femoral loss. Larger studies are needed to assess the effectiveness of alendronate in reducing fracture risk.
